Revoke, renew, or change a POA relationship

Revoke (end)

Online through MyFTB.

Mail: Send us the Power of Attorney Declaration Revocation (FTB 3520-RVK):

Phone: Call with the name(s) of the representative (s) that will be revoked.

  • From within the United States: 800-852-5711
  • From outside the United States: 916-845-6500
Estates, trusts, and group returns can only revoke by phone and mail (unless they ask their tax professional, who can do it online).

Add a representative, if authorized

Online through MyFTB.

Mail: Submit a new POA declaration. Only include new representatives on the form.

Remove a representative

Online through MyFTB.

Mail: Send us a request in writing. Include:

  • Individual or businesses’ name and ID
  • Name of representative to be removed
  • Signature date of the POA declaration to be updated

Phone: Call with the name(s) of the representative (s) that will be revoked.

Update representative's contact information

Online through MyFTB.

Mail: Send us a request in writing of the changes. Include:

  • Individual or business name and ID
  • Updated information
  • Signature date of the POA to be updated
Renew

No, you can not renew a POA. Instead, submit a new POA declaration.
