If you received this notice, we have received a tax return filed with your name and social security number. You have an opportunity to let us know if you or an authorized representative submitted the tax return on your behalf.

How to respond

Online

Use our self-service to complete the form and upload your signature document. You can access this by:

• Logging into our confirm return service.
• Using MyFTB. Do not have a MyFTB account? Create an account.

Phone

Call FTB using the phone number indicated on the notice.

Additional information

If you do not verify the submission of the tax return, we will resume processing the tax return as filed.
