Tax return missing information notice

If you received this notice, we were unable to finish processing your tax return as filed. You have an opportunity to provide the missing information before we resume processing your return. The notice specifies what information is missing and the due date for your response.

How to respond

Gather the information needed to complete the form requested on your notice and submit it, with the included Signature Authorization Document (FTB 4081) using one of the following methods.

Online

Use our self-service to complete the form and upload your signature document. You can access this by logging into MyFTB. Do not have a MyFTB account? Create an account.

Paper

Fill out the requested form and mail the documents to the address listed on the Signature Authorization Document (FTB 4081).

Additional information

If you do not send the required information by the due date of the letter, we will resume processing your tax return as filed. Processing your tax return without the requested information may result in additional tax.

If you did not get a Tax Return Missing Information (FTB 4306) notice and need to make changes to your tax return, you will need to amend your return.
