In the event a request for access to programs, services or facilities cannot be resolved with the assistance of FTB’s ADA Coordinator, a grievance may be mailed to FTB’s Equal Employment Opportunity (EEO) Office. Please refer to ADA Title II Grievance Procedures and the Grievance Form (FTB 5722). The EEO Office is available to assist disabled persons with filing a grievance.
FTB will respond to the grievance in a format accessible to the grievant such as:
- Alternative Text attribute (ALT attribute-.shtml)
- TTY (Device to Device) phone lines
- Large Print
- Audio CD Recording
We annually review assistive technology to determine the most common software and test using these tools.
