Individual: How to Submit FTB Form 3520 PIT, Power of Attorney Declaration

To Submit FTB Form 3520 PIT, Power of Attorney Declaration online, log in to MyFTB and follow the “How To” instructions below. If, once you’re logged into MyFTB, you need help with the page you’re on, select the “?” in the upper right corner of that particular page. If you need help with deciding what information to add to the online form, refer to the paper form’s instructions.

File a Power of Attorney from the Services dropdown menu

  • Review your information.
  • Select the Edit button to edit your information for the declaration, if necessary.
  • Select the Next button.

Enter Taxpayer Information

  • Enter representative information for the declaration.
    * = required field
    Note: For the POA representative to access to your MyFTB account information, we recommend you use the representative’s PTIN. For the representative to receive email notification when we send you a notice, you must enter the representative’s Email address.
  • Select the Add button.

Enter Representative Information

  • Select the Add New Representative button to add additional representatives to the declaration, if applicable.
    Note: If you have more than one representative, you must indicate which one should be the primary.
  • Select the Next button.

Add Additional Representatives

  • Select the All Years checkbox or the Specific Years checkbox.
    • If you select All Years proceed to Step 8.
    • If you select Specific Years, you will be prompted on the next page to enter the Specific Years to be covered by this POA. Proceed to Step 6.
  • Select the appropriate checkboxes for Additional Authorizations, if applicable.
  • Select the Next button.

Applicable Years and Authorizations

  • Enter the specific years to be covered by the POA.
  • Select the AddĀ button.

Enter Specific Years

  • Review specific income periods covered by the POA declaration.
  • Select Add to include additional income periods. /li>
  • Select the Next button.

Review Specific Years

  • Review the information on the Summary page.
    • To edit information use the Back button at the bottom of the page or the Edit link in the section that needs to be updated. 
  • In the Authorization of POA section, check the checkbox and enter your full name.
  • Select the Submit button.

Note: After you submit the POA, it displays as “Pending” in your authorized representative list. Your POA becomes effective after FTB review and approval. FTB will notify you when processing is complete.

  • Notice the POA Declaration number.
  • Select the Return Home button to navigate back to the homepage.
