Return Information Notice - Code OS

The amount of excess State Disability Insurance (SDI)/Voluntary Plan Disability Insurance (VPDI) reported on your original return did not match the amount shown on your amended return. Please review both your original return and your amended return for the corrected amounts.

Why did I receive this notice code?

The amount you entered on your amended tax return (Form 540X, line 19A) did not match the amount of excess California SDI or VPDI withholding on your original return. We may have previously adjusted or disallowed the amount you reported on your original return.

You can only claim a credit for excess State Disability Insurance (SDI) or Voluntary Plan Disability Insurance (VPDI) if you meet all of the following conditions:

  • You had two or more California employers during 2015.
  • You received more than $104,378 in wages.
  • The amounts of SDI (or VPDI) withholdings appear on your Forms W-2 or CA Schedule W-2.

What do I need to do?
Compare your original return and any notices we sent regarding your original return with your amended return to find the reason for the difference.

If you agree with our revisions you do not need to contact us. Please update your records.

If you do not agree, please contact us.
