We disallowed your Child and Dependent Care Expenses Credit because you did not attach form FTB 3506 to your return (federal form 2441 for tax year 2000).

Why did I receive this notice code?

For tax years 2001 and after, you must attach a completed Child and Dependent Care Expenses Credit form FTB 3506 to your return to qualify for the credit. We disallowed your credit because you did not attach this form to your return.

For tax year 2000, you must attach a completed federal form 2441, Child and Dependent Care Expenses, (or federal Schedule 2, Child and Dependent Care Expenses for Form 1040A Filers) to your return. We disallowed your credit because you either did not attach the federal form or it was incomplete.

What do I need to do?

If you agree with our revisions, you do not need to contact us. Please update your records. If your notice shows a revised balance due, please pay the amount due.

If you do not agree with our revisions, send us a copy of your California Child and Dependent Care Expenses Credit Form FTB 3506.

Note: If this is for tax year 2000, send us a copy of your Federal form 2441, Child and Dependent Care Expenses Credit, or federal Schedule 2, Child and Dependent Care Expenses for Form 1040A Filers.
