Purpose of Bulletin
To inform staff that beginning July 1, 2016, business entities will be able to submit an online revivor request on the new Revivor Assistance Request Form. We have also added information to the FTB’s website.
California business entities are mandated by statute to request a revivor in writing. We developed a web based, self service process that will expedite the revivor process for business entities that have already met their compliance requirements. It will also provide an easy way for business entities that have not yet met their compliance requirements to contact us. Ultimately, this is intended to eliminate the need for the taxpayer to call or send correspondence to begin their revivor process.
To use the online self service option, taxpayers must complete the Revivor Assistance Request Form. Go to ftb.ca.gov and search for revive to locate the “How Can I Revive My Business Entity?” webpage.
Once they complete and submit the form, we will contact the business entity within 24-48 hours.

- How to revive your suspended entity
- How to qualify for a walk-through revivor at a field office
- Suspension causes and effects
- Explanation of contract voidability
- How to purchase relief from contract voidability
