Court-Ordered Debt - Debtor’s Representative Seeking Account Information

If you represent court-ordered debt debtors, you have two options to get account information:

1. File form FTB 3520, a Power of Attorney, with us so we can disclose the account information to you.
   Note: On line 3 of the form, enter Court-Ordered Debt Collections.
   
   
2. The debtor can call us at (916) 845-4064 and give us permission to verbally disclose account information to you.
